Alyse Bynum, MBA, OTR, CHT Physical Therapy 31 Hall Drive Amherst, MA 01002 413-256-4415	Ms. Bynum is a certified Hand Therapist treating orthopedic and surgical patients for more than 20 years. She graduated Magna Cum Laude with a BS in Occupational Therapy from the State University of New York at Buffalo and earned an MBA with a concentration in Health Care Management from the University of Massachusetts, graduating Summa Cum Laude. Diane Fisher-Katz, MS, PT, CERT.MDT Physical Therapy 70 Main Street Florence, MA 01062 413-586-8400	Ms. Fisher-Katz provides physical therapy services to orthopedic patients at the Northampton Health Center. She is a graduate of Colgate University with a B.S. in Biology, and earned an M.S. in Physical Therapy from Boston University, where she also received the Rehabilitation Service Award in 1986. Ms. Fisher-Katz is certified in Mechanical Treatment and Diagnosis by The McKenzie Institute. Formerly, she was Clinic Manager at Atlantic Sports Medicine and Physical Therapy in Melrose, MA and was on staff at Braintree Hospital. Ms. Fisher-Katz has taught Orthopedic Management Lab Assistance at Springfield College. She is a member of the American Physical Therapy Association, Orthopedic section. Sandy Kobylarz , PT Physical Therapy 238 Northampton Street Easthampton, MA 01027 413-282-3853	Ms. Kobylarz graduated with honors from Boston University. She comes to us from Cooley Dickinson Hospital where she has served the Pioneer Valley for 19 years. Sandy began her career in NJ where she implemented the first cardiopulmonary rehabilitation program at Somerset Medical Center. She has also participated in educational series addressing the role of exercise in the management of Diabetes Mellitus. Currently, Sandy has a special interest in assessing and treating shoulder and lower extremity injuries.  She utilizes a detailed biomechanical assessment of the lower kinetic chain to design effective treatment interventions  and maximize patients function.
Michelle Kofler, MS, PT Physical Therapy 329 Conway Street Greenfield, MA 01301 413-774-6301	Ms. Kofler graduated with a Master of Science in Physical Therapy from Springfield College, Springfield, Massachusetts and a Bachelor of Arts in Sociology from the University of Massachusetts in Amherst.  Her expertise includes the evaluation and treatment of patients with a wide variety of orthopedic diagnoses.  Ms. Kofler has a special interest in foot and lower extremity biomechanical evaluation and she performs foot casting for custom orthotic fabrication.  She previously worked at University of Massachusetts in the Health Center Physical Therapy Department.  Ms. Kofler joined the Valley Medical Group in September of 2000. Susan Looze, P.T. Physical Therapy 31 Hall Drive Amherst, MA 01002 413-256-4415	Ms. Looze has been a physical therapist for more than 20 years. She graduated from Simmons College with a B.S. in physical therapy. Ms. Looze works with outpatient orthopedic patients at the Amherst Medical Center. Her previous work experience includes Boston Children's Hospital, Champaign County Nursing Home, Champaign Children's Home, Greenfield Public Schools, and Olympus Specialty Hospital in Springfield. NanciAnn Machnik, PT Physical Therapy Dept. 329 Conway Street Greenfield, MA 01301 413-774-6301	Ms. Machnik a 1979 graduate of the University of Pennsylvania, Philadelphia, PA  Physical Therapy program.  She has relocated to the Pioneer Valley from Eastern Mass. where she worked most recently at New England Sinai Hospital.  Previous to that, she was a Senior Clinician in Milton, MA, and also Clinical Instructor in Spinal Manual Therapy at Braintree Rehabilitation Hospital in Braintree MA.  She has a particular interest and advanced coursework in osteopathic manual therapy.  Ms. Machnik has been a member of the American Physical Therapy Association since 1978 and is currently a member of the  Sports Physical Therapy Section of the APTA. O'Sullivan, PT, OCS, ATC Physical Therapy 70 Main Street Florence, MA 01062 413-586-8400	Mr. O'Sullivan is a physical therapist with board certification in Orthopedic Physical Therapy from the American Board of Physical Therapy Specialists. He has a B.S. from the State University of New York at Buffalo. Mr. O'Sullivan has worked throughout the United States. Locally, he has been a member of the Rehabilitation departments at the Cooley Dickinson Hospital and at Mercy Hospital in their Industrial Rehabilitation Program. He is also a certified athletic trainer and has received certification in mechanical diagnosis and therapy of the spine. He is a member of the American Physical Therapy Association, the National Athletic Trainers Association and the McKenzie Institute USA. Christopher Peterson, DPT Physical Therapy 238 Northampton Street Easthampton, MA 01027 413-282-3853	Mr. Peterson received his Masters Degree in Physical Therapy from American International College in Springfield, MA. He comes to us from Farnum Rehabilitation Center, a hospital based outpatient orthopedic clinic in Keene, NH where he set a standard in providing quality evidence-based assessment and care of musculoskeletal conditions. He has most recently completed his Doctorate in Physical Therapy from Temple University and received a post-graduate Certificate in Mechanical Diagnosis and Therapy from the McKenzie Institute USA. Chris has a special interest in orthopedic physical therapy for patients of all ages, using an education centered approach to treatment.
John L. Prinzivalli, P.T., M.S. Physical Therapy 31 Hall Drive Amherst, MA 01002 413-256-4415	Mr. Prinzivalli has more than 20 years experience as a Physical Therapist with special emphasis in the treatment of low back pain. He graduated with a B.S. from the State University of New York at Stony Brook and got his Masters Degree from the University of Scranton. Mr. Prinzivalli has instructed and lectured on physical therapy and orthopedic health issues, and was a 1998 presenter to the Institute for Healthcare Improvement, National Congress on Improving Care for Low Back Pain. He is a member of  the Massachusetts Physical Therapy Association, and the Orthopedic Section of the American Physical Therapy Association. Caroline White, MPT Physical Therapy 329 Conway Street Greenfield, MA 01301 413-774-6301	Prior to Caroline joining the Physical Therapy Dept at Greenfield Health Center in May 2004, she worked with Baystate Rehabilitation Services at Franklin Medical Center.  Ms. White has her Masters Physical Therapy from American International College in Springfield, Massachusetts.  She is a member of the American Physical Therapy Association, the Massachusetts Physical Therapy Association, and the Orthopedic Section of the American Physical Therapy Association.  She brings skills in general and orthopedic physical therapy as well as a Certification in Vestibular Rehabilitation.
