Come and join a multidisciplinary team working in collaboration with the Diabetes Education Program Director to deliver care management services, education, and direct patient care to our members with diabetes. Advancement to a management position possible for the right candidate. Minimum of 2 years of experience as a Registered Nurse with a valid Massachusetts license required; Minimum of 1 years Experience in Diabetes Education as CDE; Membership in the American Diabetes Association and/ or the American Association of Diabetes Educators required; Must possess certification from the National Board of Diabetes Educators (CDE); Experience in an outpatient setting required; Care management and electronic medical record experience are preferred.
